Job summary

BJC HealthCare is seeking a Hospice - Registered Nurse Case Manager to provide compassionate, patient‑centered home hospice care across the St. Louis region. Schedule: Monday–Friday, 8:00am–4:30pm, with weekend and on‑call rotations; must be willing to cover all St.

Louis regions during on‑call. The role emphasizes compassionate, culturally sensitive care and requires nursing licensure and a valid driver’s license.

Qualifications

• Nursing diploma or an associate degree in nursing is required.
• Valid Driver's License and RN licensure.
• Preferred: 2–5 years of nursing/home health experience; leadership/supervisor experience.

Responsibilities

• Assess patient needs and develop care plans with the interprofessional team.
• Coordinate care across the continuum and communicate plans to patients and families.
• Promote equity, empathy, and patient-centered care in diverse populations.
• Document plans and ensure evidence-based nursing care and quality outcomes.
