Job summary

BJC Home Care is seeking a Hospice - Registered Nurse Case Manager to cover patient care in the St. Louis area. The role focuses on evaluating clients, coordinating all aspects of care, and ensuring effective communication among the interprofessional team, patients, and families.

Responsibilities include developing individualized plans of care, supporting emotional and cultural needs, and ensuring up-to-date documentation in the EMR. This is a safety-sensitive position with on-call coverage.

Qualifications

• Nursing Diploma/Associate's required.
• RN licensure and valid driver's license preferred.
• Bachelor's degree in nursing/home health is preferred.

Responsibilities

• Assess patient needs and preferences and coordinate care.
• Develop and document care plans with the interprofessional team and family/caregiver.
• Promote equity and empathy in care, supporting emotional, spiritual, and cultural needs.
• Evaluate changes in patient condition and update plans in the EMR.
• Maintain safety and alertness in a safety-sensitive role.
