Phoenix Home Care and Hospice in St. Louis, MO is seeking a dedicated RN Case Manager to deliver compassionate hospice nursing care to clients and families within the interdisciplinary team framework.

The role involves planning and evaluating care plans, guiding families through goals of care discussions, coordinating with physicians, and educating patients and caregivers, with a Monday–Friday daytime schedule.

Qualifications

• Missouri RN License required (or Compact).
• Valid driver's license and auto liability insurance may be required.

Responsibilities

• Provide education to patients/family members regarding the process and plan of care.
• Consult with attending physician and Hospice Medical Director on changes to patient condition or plan of care.
• Coordinate hospice care with team members to meet patient needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
