UnitedHealth Group, part of the Optum family, is seeking an RN Case Manager to coordinate compassionate hospice care. You will assess patients, partner with the care team, and develop individualized plans to support each patient’s needs.

Salary ranges from $79,600 to $119,400 annually, with a $7,500 sign-on bonus for external candidates. Benefits include comprehensive packages and 401k contributions. Equal opportunity employer.

Qualifikationen

• Current RN license, state(s) you are assigned to work.
• Current CPR certification.
• 1+ year of nursing experience.
• Valid driver's license with access to reliable transportation and liability insurance.

Aufgaben

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
