Job summary
Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. You’ll be part of an interdisciplinary team dedicated to The Best Care Possible in Berkeley, Charleston, and Dorchester counties.
The role offers a Monday–Friday schedule, 8 am–4:30 pm, and a $7,500 sign-on bonus.
Qualifications
- Hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
- Computer proficient with EMR; CPR as required by state.
- Valid driver’s license, vehicle insurance, and reliable transportation.
Responsibilities
- Provide hands-on hospice nursing care and coordinate services.
- Guide patients and families through the hospice journey.
- Collaborate with the interdisciplinary care team to ensure quality care.
- Educate and advocate for patient goals and comfort.
- Participate in call duties as needed.
