Blue Ridge Care is seeking an RN Case Manager for hospice care to join our compassionate team. This full-time role focuses on direct patient care, coordinating individualized care plans, and serving as a trusted presence for patients and families.

You will collaborate with an interdisciplinary team to ensure comprehensive care for body, mind, and spirit, delivering skilled nursing and symptom management with a strong emphasis on comfort and dignity.

Qualifications

• Must hold a current RN license in Virginia (multi-state or single-state).
• CPR certification and at least two years of nursing experience (hospice or home health preferred).
• Strong clinical judgment, communication, and organizational skills.
• Reliable transportation, valid driver’s license, and insurance.
• A self-directed, compassionate spirit ready to serve your community.

Responsibilities

• Assess patient and family needs and develop personalized care plans.
• Deliver skilled, compassionate nursing care and symptom management.
• Administer medications and coordinate care across the healthcare team.
• Educate and support families to promote comfort and peace of mind.
• Lead and guide hospice aides to ensure consistent, compassionate care.
• Maintain accurate documentation and participate in ongoing education.
