Job summary

Amedisys, part of the Optum family, seeks an RN Case Manager to coordinate hospice care by assessing patients and partnering with the interdisciplinary care team to develop plans that meet each patient’s needs. This role emphasizes compassionate care, collaboration, and accurate documentation in a patient-centered environment.

You will lead patient assessments, document outcomes, educate families, and communicate with the care team while supporting on-call responsibilities and adherence to care

Qualifications

• Current RN license valid in the assigned state(s).
• Current CPR certification required.
• 1+ years of nursing experience.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
