Job summary

Chapters Health System seeks an experienced RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative, and supportive care to keep patients at home whenever possible.

You will complete initial and semi-annual assessments, educate families, initiate plans of care, and collaborate across IDT to ensure quality care and timely communications.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Florida Alzheimer’s Disease and Related Dementias Training for PACE employees.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants at their homes for assessments.
• Ability to meet physical requirements (lifting, bending, etc.).
• Active BLS certification for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT with psychosocial, spiritual, cultural, physical and biological components.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, palliative care, caregiving, safety and decline.
• Frequent IDT communication and SBAR-based clinical updates.
• Home visits to assess safety, medication and DME compliance and overall living conditions.
• Participates in IDT to evaluate hospice referrals/admissions for level of care.
• Perform other duties as assigned.
