Qualifications

• Reliable transportation required for home visits.

Responsibilities

• Coordinates with the Interdisciplinary Team (IDT) for patient needs.
• Conducts home assessments for safety and compliance.
• Documents patient interactions in the EMR and communicates with the IDT.
