Job summary

Chapters Health System is seeking an experienced RN Case Manager to assess patient/family needs, coordinate the Plan of Care with the IDT, and provide clinical and palliative care in the patient’s home environment to maximize comfort and independence.

The role requires an active RN license, 1+ year nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems. A valid driver’s license and reliable transport are essential; benefits start on day 1.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• Alzheimer's Disease and Related Dementias Training for PACE Florida.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and auto insurance; reliable transportation.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants at home for assessments.
• Physical requirements including lifting and mobility.
• Active BLS for healthcare professionals (AHA/Red Cross).

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting evolving needs.
• Educates patients and families about disease, palliation, safety and care.
• Home visits to assess safety, medications, nutrition, and DME.
• Reports changes to IDT and physicians; SBAR communications.
• Participates in referrals/admissions and collaboration with facilities.
• Oversees care delivery and coordinates with CHWs and HHAs.
