WesleyLife At Home Hospice in Urbandale, IA is seeking a dedicated Hospice RN Case Manager to coordinate patient care in home settings and support families through the hospice journey. As an RN Case Manager you will travel to patients, assess needs, manage symptoms, develop care plans with the inter-disciplinary team, educate caregivers, and provide compassionate presence during difficult times.

Qualifications

• Must have current vehicle insurance.
• Must hold an Iowa RN license.
• Minimum two years of full-time nursing experience; hospice experience preferred.

Responsibilities

A Typical Day for a Registered Nurse Case Manager with WesleyLife At Home Hospice:

As an RN Case Manager with WesleyLife at Home Hospice, each day begins with purpose—the opportunity to bring comfort, dignity, and compassionate support to patients and families navigating one of life’s most meaningful journeys. During your visits as a Registered Nurse Case Manager you will:

Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well‑being

Manage pain and other symptoms using evidence‑based hospice practices

Coordinate medications, treatments, and supplies to support patient needs

Prepare and update individualized care plans in collaboration with the interdisciplinary team

Provide education on what to expect throughout the hospice journey

Teach family caregivers how to provide safe, comfort‑focused care

Offer a listening ear and calm presence during moments of uncertainty

• Full-Time Monday-Friday, Back-up on-call Weekday and weekends

Qualifications:

• Current vehicle insurance.
• Current license as a registered nurse in the state of Iowa.
• Two years of full-time nursing experiencerequired, with one year of hospice experience preferred.
