WesleyLife Hospice seeks a Registered Nurse Case Manager to provide in‑home hospice care, travel to patient homes, and collaborate with an interdisciplinary team to monitor symptoms and comfort. The role emphasizes compassionate, patient‑ and family‑centered care across diverse settings.
The ideal candidate holds an Iowa RN license, has at least two years of full‑time nursing experience (one year hospice preferred), and possesses a valid Iowa driver’s license with vehicle insurance.
