Pathways Hospice (Newest Start Up) in Texas is seeking a licensed Registered Nurse to join our hospice team on a PRN basis. You will oversee nursing care, collaborate with physicians and therapists, and coordinate home-based services for patients and families.

The role emphasizes independent work and teamwork, patient education, timely documentation, and availability for night and weekend coverage as hospice needs dictate.

Qualifications

Must have an active RN license in Texas and hospice experience.
Knowledge of home health regulations and compliance.
Experience with Home Care Home Base (HCHB) EMR is preferred but not required.
Excellent communication and people skills are essential.
Ability to work independently and as part of a team.

Responsibilities

Oversee nursing care team.
Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
Plan, organize, and direct home care services.
Conduct assessments of patient care needs and develop care plans.
Educate patients and their families on managing health conditions, medication regimens, and necessary lifestyle changes.
Prepare timely, complete, and accurate clinical notes.
Night and weekends are required per hospice needs of patients.
