Job summary

Heart to Heart Hospice is seeking an experienced RN Case Manager for PRN weekend coverage in Texas. You will perform comprehensive assessments and develop individualized plans of care for patients facing terminal illness.

Requirements include an accredited nursing degree, active RN license in Texas, a valid driver's license with reliable transportation, and at least two years of RN experience. This role includes on-call duties and benefits.

Qualifications

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current Driver's License with reliable transportation.

Responsibilities

Initial and ongoing comprehensive assessments of the patient’s physical, psychosocial, and environmental needs.
Implement the individualized Plan of Care (POC) and revise as necessary.
Collaborates with patient, caregiver, physicians and other healthcare professionals.
Attend Inter-Disciplinary Group (IDG) meetings and coordinate care planning.
Perform on-call responsibilities and provide on-call service to patients/families.
Timely submission of all documentation and ensure regulatory compliance.
Perform other duties as required.
