Pathways Hospice (Newest Start Up) in Texas is seeking a licensed Registered Nurse to join our hospice team on a PRN basis. You will oversee nursing care, collaborate with physicians and therapists, and coordinate home-based services for patients and families.

The role emphasizes independent work and teamwork, patient education, timely documentation, and availability for night and weekend coverage as hospice needs dictate.

Qualifications

• Must have an active RN license in Texas and hospice experience.
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR is preferred but not required.
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.

• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions, medication regimens, and necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and weekends are required per hospice needs of patients.

Registered Nurse (RN) - Hospice

Shift Available: PRN

Service Area for Coverage: Northwest, Southwest & Southeast

Responsibilities
