Job summary

Halcyon Home LLC in Austin, TX is seeking a compassionate Hospice Registered Nurse (RN) to join our team on a PRN basis. You will conduct patient assessments, collaborate on care plans, and provide education to patients and families in a home-health and hospice setting.

Ideal candidates hold an active RN license, have home health or hospice experience, and bring strong communication and time-management skills.

Qualifications

• Current RN license in good standing with the state nursing board.
• Experience using Homecare Homebase EMR is a plus.
• Hospice or related healthcare experience preferred.
• Strong clinical, assessment, and communication skills.

Responsibilities

• Conduct comprehensive patient assessments and admissions.
• Collaborate with the interdisciplinary team to develop and implement care plans.
• Offer support and education to patients and their families.
