Job summary

Pathways Hospice is seeking a Registered Nurse (RN) for PRN shifts to provide compassionate in-home hospice care across Northwest, Southwest and Southeast Texas, from the Houston area.

You will oversee the nursing care team, collaborate with physicians and other professionals, plan and direct home care services, educate patients and families, and document clinical notes. Night and weekend coverage are required to meet patient needs.

Qualifications

• Must hold a valid Texas RN license.
• Hospice experience (minimum 1 year) preferred.
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base EMR is preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals.
• Plan, organize, and direct home care services.
• Conduct assessments and develop care plans.
• Educate patients and families on conditions and medication regimens.
• Prepare timely, complete clinical notes.
• Night and weekends required per hospice needs.
