Job summary

Luminary Hospice in Indiana seeks a PRN Registered Nurse Case Manager to plan, organise, and direct hospice care in patients' homes and communities. The role emphasizes comprehensive assessments, care planning, and symptom management, guided by a patient- and family-centered approach.

The RN will coordinate with the interdisciplinary team, provide education, supervise aides, and participate in on-call duties as defined by policy. Indiana RN license and CPR are required.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree; hospice care experience preferred.
• Management experience is not required, but supervising hospice aides may be required.
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Completes initial, comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Assists with activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager as assigned by Clinical Supervisor and coordinates patient care.
• Maintains accurate clinical notes and communicates with physicians and care team.
• Participates in on-call duties and ensures necessary equipment and services are available.
• Supervises ancillary personnel as required.
