Luminary Hospice in Fort Wayne, IN is seeking a PRN Registered Nurse Case Manager to organize and direct hospice care in patients’ homes and communities. The role emphasizes nursing expertise, community health education, and coordinating interdisciplinary care to meet patient and family needs.

Qualifications

Graduate of an accredited nursing program with an active Indiana RN license.

Strong assessment and care planning skills.
Excellent communication with patients, families, and clinical teams.

Responsibilities

Completes comprehensive patient and family assessments to determine hospice needs.
Develops and revises a patient-specific plan of care in collaboration with physicians and the care team.
Administers medications and coordinates symptom management per physician orders.
Provides patient and family education on care needs and end-of-life planning.
Coordinates services, on-call duties, and supports home-based care in the community.
Maintains accurate clinical documentation and communicates changes to the team.
