Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree with hospice care experience preferred.
• Management experience is not required; capable of supervising aides.
• Excellent communication and nursing skills per competency checklist.

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises a care plan with the team and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Coordinates patient care as Case Manager with the interdisciplinary team.
• Maintains accurate clinical notes and documents changes in patient status.
• Communicates with physicians and community health professionals to coordinate care.
• Provides patient/family education and self-care guidance.
