Job summary

Legacy Hospice is seeking a Per Diem Hospice RN Case Manager to join our dedicated nursing team in Charlottesville. You will provide hands-on hospice nursing care, coordinate services, and guide patients and families through the hospice journey with compassion and clinical excellence.

The role requires an active RN license and at least one year of Med/Surg/acute care experience, plus EMR proficiency and a valid driver's license.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting.
• Must be computer proficient in typing and EMR systems.
• Current CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver direct hospice care and coordinate services for patients and families.
• Provide nursing care rooted in compassion and clinical excellence.
• Educate patients and families to support personalized care plans.
• Join rotating on-call duties to ensure timely support.
