Presbyterian Healthcare Services in Albuquerque is seeking a Registered Nurse (RN) to provide direct patient care and case management for Hospice patients in outpatient settings, ensuring high-quality end-of-life care aligned with clinical guidelines.

Qualifications

• Associate Degree in Nursing.
• State of New Mexico or Compact State Nursing License.
• BLS certification REQUIRED at time of hire.
• One year in Direct patient care setting or home health care.
• Hospice experience preferred.
• Current driver’s license and auto insurance.

Responsibilities

• Sign on and relocation benefits available to qualified candidates.
• Provide direct patient care and case management of Hospice patients in outpatient settings.
• Coordinate and schedule care through a multidisciplinary team in home or community settings.
• Maintain timely, accurate, and reimbursable documentation.
• Precept newly hired nurses and serve as a resource.
• Participate in QAPI and other improvement activities.
