Responsibilities

• Complete initial patient/family assessment to determine hospice care needs.
• Regularly re-evaluate nursing needs of patients.
• Use assessment data to determine care plan needs.
• Collaborate with healthcare team, patient, and family for coordinated care.
• Counsel patient and family on end-of-life needs.
• Initiate preventative and palliative nursing procedures.
• Communicate with physician about patient needs and changes.
• Supervise Hospice Aides.
