Job summary

Ascend Hospice & Palliative Care VA is hiring an RN Case Manager - Per Diem to support on-call coverage, case management, and patient visits. You will assess patients and families to determine hospice needs and collaborate with the healthcare team to coordinate continuous, high-quality care.

Required: Associate's Degree in Nursing, state RN license, 2 years nursing experience (hospice preferred), valid driver's license and willingness to travel.

Qualifications

• Associate's degree in nursing and RN license required.
• 2 years of professional nursing experience; hospice experience preferred.
• Valid driver’s license and ability to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient’s nursing needs and adjust care plans.
• Collaborate with the healthcare team to coordinate continuous, well‑led care.
• Counsel patient and family in meeting end‑of‑life needs.
• Initiate preventive and palliative nursing procedures as appropriate.
• Communicate with the physician about patient needs and changes in condition.
• Supervise the Hospice Aides.
