Qualifications

• Licensed registered nurse currently licensed to practice in Louisiana.
• At least two years of full-time RN experience; hospice experience may substitute for RN experience.
• Must coordinate care with the patient, family, and other IDT members and document care.

Responsibilities

• Provide nursing services per the POC.
• Document problems, goals, interventions, and patient response.
• Collaborate with attending physician and health-care team.
• Instruct patient/family in self-care techniques when appropriate.
• Supervise aides and delegate tasks as needed.
• Submit clinical notes within one week of visits.
• Supervise and evaluate hospice aide performance.
• Conduct on-site supervisory visits every 14 days and annual aide reviews.
