Job summary

Responsibilities include conducting evaluations, integrating care plans, and documenting patient responses. Qualifications include a current RN license, driver’s license and access to transport. Comprehensive benefits and salary ranges from $75,620 to $113,430 annually.

Qualifications

• Must hold a current and unrestricted RN licensure in the state of practice.
• Experience in hospice care preferred.
• Valid driver's license with a reliable vehicle or access to public transportation.

Responsibilities

• Conduct initial nursing evaluation for hospice services within 48 hours of referral.
• Identify and reassess the patient's physical, psychosocial, and environmental needs.
• Document problems, goals, interventions, and patient/family responses.
