Job summary
Presbyterian Healthcare Services is seeking an experienced Registered Nurse to join our Outpatient Hospice department. You will provide direct patient care and case management for hospice patients in outpatient settings, ensuring end-of-life care aligns with quality and utilization guidelines and improves comfort and quality of life.

Qualifications
- Associate Degree in Nursing; State of New Mexico or Compact State Nursing License.
- BLS certification REQUIRED at time of hire.
- One year in direct patient care setting or home health care; Hospice experience preferred.
- Current driver's license and auto insurance.
Responsibilities
- Provide direct patient care and case management for Hospice patients in outpatient settings.
- Coordinate care through a multidisciplinary team in home or community settings.
- Assure completeness, timeliness and appropriateness of the patient care plan and ongoing evaluation.
- Maintain timely documentation and communication with the team to ensure goals are met.
