• 1 year of hospice experience required.

• Current driver's license and auto insurance required.
• BLS certification required.
• RN licensure in New Mexico (RN) required.
• Associate's degree in nursing from an accredited program.

Responsibilities

• Bridge referrals sources and coordinates with Hospice/Palliative Care teams to determine the right program and level of care.
• Evaluate complex medical data to ensure accurate patient-program matches and smooth transitions.
• Lead case consultations and intake triage to map goals of care and appropriate resources.
