Job summary

Anchor Health is seeking a Hospice RN Case Manager to initiate and coordinate comprehensive hospice care plans for patients in home and community settings. The role emphasizes pain management, symptom control, and interdisciplinary collaboration to support patients and families facing terminal illness.

Responsibilities include regular plan-of-care updates, nursing assessments, patient and family education, and serving as the primary case manager for a defined caseload.

Qualifications

• RN License is required.
• Hospice experience preferred (at least 1 year).
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Inits and coordinates the hospice care plan; organizes and directs hospice care.
• Reviews and updates the plan of care; performs prescribed medical treatments and pain management.
• Educates and supports patient and family; documents assessments and changes.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Communicates with physicians and community health professionals to coordinate care.
• Provides self-care instruction to patient/family and ensures safety in the home environment.
