Job summary

ANCHOR HEALTH is seeking a Hospice RN Case Manager to coordinate end-of-life care for patients in home and community settings. The role centers on nursing leadership, care plan development, and symptom management with strong family education and support.

The ideal candidate holds a CA RN license, CPR, and at least one year of hospice or acute care experience, with reliable transportation for on-the-road visits. Schedule: Monday–Friday with potential flex. Pay is based on experience.

Qualifications

• RN license valid in California.
• Hospice experience preferred and beneficial.
• CPR certification current and maintained.
• Must have reliable transportation and driver’s license.

Responsibilities

• Provide hospice care and coordinate the care plan for assigned patients.
• Regularly review and update the plan of care.
• Administer prescribed treatments and manage pain and symptoms.
• Complete comprehensive assessments of patient and family needs.
• Develop and implement a care plan with goals and nursing actions.
• Communicate with physicians and the interdisciplinary team.
• Educate patient and family on self-care and care options.
• Ensure safe home environment and continuity of care.
