Job summary

Ascend Hospice & Palliative Care MA seeks an RN Case Manager to support on-call, case management, and patient visits in the hospice program. You will assess patients and families, develop care plans, and collaborate with the healthcare team to ensure continuous, well-coordinated care.

The role requires an active RN license, 2 years nursing experience (hospice preferred), the ability to travel, and strong communication skills. We offer competitive compensation and benefits.

Qualifications

• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family, to coordinate continuous and well‑coordinated care.
• Counsels the patient and family in meeting end-of-life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient needs and reports changes in condition.
• Supervises the Hospice Aides.
