Job summary

Ascend Hospice & Palliative Care MA is seeking a Hospice Registered Nurse Case Manager to support on‑call coverage, case management, and patient visits. You will assess patient needs, develop care plans, and coordinate with the healthcare team to deliver continuous and compassionate care.

The ideal candidate holds an active RN license with at least 2 years of nursing experience, hospice preferred. A valid driver’s license and willingness to travel are required.

Qualifications

• Active RN license required.
• 2 years nursing experience; hospice experience preferred.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial patient and family assessments to determine hospice care needs.
• Regularly reevaluate patients' nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family for coordinated care.
• Counsel patient and family in end-of-life needs.
• Initiate appropriate preventative and pallaitive nursing procedures.
• Communicate with the physician about patient needs and changes in condition.
• Supervise the Hospice Aides.
