Job summary

Anchor Health is seeking a compassionate Registered Nurse to lead hospice care in home settings, coordinating with families and physicians. You will assess patient needs, develop and revise care plans, and liaise with the Interdisciplinary team to ensure comfort and quality of life.

Requirements include CA RN license, CPR certification, and 1 year of recent nursing experience; 2+ years in home health or hospice preferred.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current California RN license and CPR certification.
• 1 year of nursing experience in the last 3 years, or a BSN with RN license.
• Two years of experience in public health, home care, or hospice nursing preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Provide professional nursing care to hospice patients in home settings using the nursing process.
• Assess and evaluate patient status; initiate and revise the plan of care as needs change.
• Coordinate patient care with the Interdisciplinary team and community physicians; communicate patient needs.
