Job summary

Heartland Hospice seeks a dedicated RN Case Manager to lead hospice care in patients' homes, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to deliver personalized plans of care.

You will perform comprehensive assessments, provide skilled nursing interventions, educate families, and document care activities while ensuring quality and compliance in a compassionate setting.

Qualifications

• Registered Nurse licensed to practice in the state of employment.
• Minimum of 1 year RN experience in hospice, home health, oncology, long-term care, or acute care.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educating patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care, including visit frequency.
• Coordinate care across interdisciplinary teams with physicians and nurses.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications, nutrition.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care and transitions between home health, palliative, and hospice levels.
• Document timely clinical records including assessments, orders, progress notes and care plans.
• Maintain relationships with the IDG and involve the team as needed.
• Participate in on-call and patient support activities as applicable.
