Job summary

Qualifications

• RN license in state of employment.
• At least 1 year of RN experience in hospice, home health, or related settings preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic visits.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with physicians and IDG members.
• Educate patients and caregivers on disease, self-care, and end-of-life needs.
• Document timely clinical records and maintain compliance.
