Job summary

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team and develop the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and staff.

You will provide day-to-day patient care, administer medications, and ensure accurate records and compliance. You’ll coordinate with physicians and an interdisciplinary team, ensuring a safe home environment and compassionate support for patients and families.

Qualifications

• Current RN license in state(s) of practice.
• Multistate licenses acceptable where allowed.
• Background check, DHS/consumer report, and drug test may be required.
• Valid State Driver’s License and reliable transportation are required.
• Experience in hospice/home care is preferred.

Responsibilities

• Lead patient’s hospice care team and develop the Plan of Care with the Medical Director, patient, and family.
• Provide direct patient care, administer medications, and document care activities.
• Coordinate with physicians, interdisciplinary teams, and community partners.
• Maintain HIPAA compliance and accurate clinical documentation.
• Participate in on-call visits, meetings, and ongoing professional development.
