Qualifications

• Must hold an active RN license in state(s) of practice.
• Graduate of a professional school of nursing.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and access to a vehicle with proper insurance.

Responsibilities

• Lead the hospice care team to implement the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.
• Provide day-to-day patient care and administer medications as prescribed.
• Maintain accurate clinical documentation, assess pain and symptoms, and coordinate care with physicians and teams.
• Offer emotional support and counseling to patients and families.
• Participate in on-call visits and attend interdisciplinary group meetings.
