Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and tailor the Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff.

You will provide day-to-day nursing care, manage medications, maintain records and guide other team members to meet evolving patient needs, focusing on comfort and quality of life in a home setting.

Qualifications

• Must have a current RN license in state of practice.
• Multistate license acceptable; primary state residency rules apply.
• Graduate of a professional nursing program and licensure.
• Valid State Driver’s License and reliable automobile.
• Background check and drug screen required.
• On-call availability including evenings and weekends.

Responsibilities

• Lead patient hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, family and facility staff.
• Provide day-to-day patient care and administer medications per physician’s plan.
• Educate patients and families on self-care, medications, and diet as appropriate.
• Maintain accurate clinical documentation of pain, symptoms and changes in condition.
• Coordinate care with physicians, interdisciplinary teams, and community partners.
• Participate in quality assessment and performance improvement activities.
