Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team, developing and managing the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RN Case Manager provides day-to-day patient care, administers medications, maintains patient records, and directs other members of the patient care team to meet changing needs, focusing on comfort and quality of life in home settings.

Qualifications

• Current RN license in state(s) of practice.
• Experience in hospice/home care preferred.
• Ability to pass DHS background, consumer report, and drug screen.

Responsibilities

• Lead the patient’s hospice care team and develop/manage the Plan of Care with the Hospice Medical Director, patient, family, and facility staff.
• Provide day-to-day patient care, administer medications, and maintain accurate records.
• Coordinate care with physicians, interdisciplinary teams, and community partners; ensure HIPAA compliance.
