Job summary

LHC Group's Louisiana Hospice and Palliative Care is hiring an RN Case Manager to oversee the Plan of Care for hospice patients. You will assess needs, develop care plans with multi-disciplinary teams, and implement treatment strategies that support physical, psychosocial, emotional, and spiritual well-being.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care and document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG members to provide care daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes regarding the patient's condition.
