Job summary

Affinity Hospice Holdings, LLC is seeking a registered nurse/case manager to plan, organize, and direct hospice care in patients' homes and communities. You will apply nursing expertise with emphasis on education and coordination of services with families and caregivers.

The role requires current RN license in Georgia, CPR, and experience in home health or hospice. On-call weekends are required, and you join a nationally recognized team dedicated to compassionate end-of-life care.

Qualifications

• Graduate of accredited nursing program (Diploma/Associate/Bachelor).
• Current RN license in state of Georgia.
• Current CPR certification.
• Two years RN experience in clinical care/home health/hospice preferred.
• Must have vehicle with valid driver’s license.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document patient plan of care with infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for payer requirements and compliance.
• Perform on‑call duties as assigned and supervise hospice aides and LPNs.
