Job summary

Affinity Hospice in Claxton, GA is seeking a Registered Nurse Case Manager to coordinate patient care in home and community settings. You will guide an interdisciplinary team, implement the plan of care, and ensure timely, quality service.

A current RN license and CPR certification are required; driving in the community is expected. The role includes on-call coverage and promoting hospice standards in line with OSHA guidelines. Affinity Hospice is an equal opportunity employer.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current State RN license.
• Current CPR certification.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Assure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements; on-call service as assigned.
