Job summary

Affinity Hospice is seeking a full-time RN Case Manager in Thomasville, GA to plan, organize, and direct hospice care for patients at home and in the community. The role emphasizes nursing expertise, family collaboration, and adherence to care plans.

The ideal candidate holds an RN license, has at least two years in clinical care or hospice, and can manage interdisciplinary teams while maintaining documentation and compliance. Valid driver’s license required and on-call abilities may apply.

Qualifications

• Graduate of an accredited Nursing program (Diploma/Associate/Bachelor).
• Current State RN license.
• Minimum two years RN experience in clinical care, home health, or hospice preferred.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop, and document the patient plan of care.
• Provide care using infection control measures to protect staff and patients (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third party payer requirements and compliance.
• Supervise hospice aides and licensed practical nurses; perform on-call duties as assigned.
