Job summary

By The Bay Health is seeking a Registered Nurse to serve as a Hospice Nurse Case Manager. You will provide skilled nursing assessment, planning and care in the patient’s home to maximize comfort and health for patients and families.

This full-time role offers onboarding, mentoring by experienced RNs, and a collaborative team across multiple disciplines. California licensure and pediatric experience are pluses; a strong commitment to quality care is essential.

Qualifications

• AA or B.S. degree in Nursing from an accredited Nursing School.
• Current California RN license and PHN/HPNA certifications preferred.
• Experience in hospice or home health care is preferred but not required.

Responsibilities

• Provide skilled nursing assessment, planning and care in the home as part of an interdisciplinary team.
• Coordinate patient/family services and implement the individualized plan of care.
