Job summary

Bristol Hospice - Pathways Llc in Texas is seeking an experienced Registered Nurse Case Manager to provide compassionate, whole-person hospice care. You will develop individualized care plans, conduct in-depth assessments, monitor vital signs, administer physician-ordered treatments, and support patients and families with dignity and comfort.

You will collaborate with interdisciplinary teams, document progress, educate families on symptom management, and respond quickly to changes in condition.

Qualifications

• Active Texas RN license.
• CPR certification (current).
• Valid driver’s license and reliable transportation.
• Strong observational and written communication skills.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families with dignity.
• Assess physical, emotional, and spiritual needs throughout the journey.
• Monitor vital signs and document progress in clinical notes.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
