Job summary

Bristol Hospice - Michigan LLC is seeking a dedicated RN Case Manager to join our mission-driven team in Michigan. You will deliver compassionate, holistic end-of-life care and advocate for patients and families throughout the care journey.

You’ll develop individualized care plans, monitor physical, emotional, and spiritual needs, coordinate with interdisciplinary teams, and educate families on symptom management.

Qualifications

• Active RN license in good standing in the state you will work in.
• Current CPR certification.
• Strong observational and written/verbal communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort and dignity.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor vital signs and document patient progress accurately.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
