Job summary

Mission Healthcare is seeking a qualified Registered Nurse for full-time home health care in California. The role requires an active RN license, 1 year of acute care experience, BLS/CPR, and a valid driver’s license.

Qualifications

Current and valid RN license in the appropriate state.
Minimum of 1 year of recent acute care experience in an institutional setting.
BLS certification with hands-on CPR.

Bachelor’s degree in Nursing; home health experience preferred.

Responsibilities

Completes an initial assessment of the patient and family to determine home care needs.
Initiates the plan of care and makes revisions as patient status changes.
Regularly re-evaluates patients’ needs and diagnoses.
Develops care plans based on nursing diagnoses and collaborates with patient and family.
Administers medications and treatments as prescribed by the physician.
Provides health care instructions and discharge planning support.
