Job summary

Mission Healthcare, based in California, seeks an experienced Registered Nurse for our home health team serving the San Diego area. You will perform initial patient assessments, develop and revise care plans, and work with families to achieve optimal home care outcomes.

Requirements include an active California RN license, at least 1 year of acute care, BLS, a valid driver's license, and a commitment to compassionate, patient-centered care.

Qualifications

• Active California RN license.
• Minimum 1 year of acute care experience in an institutional setting.
• BLS with CPR certification.
• Valid driver's license and auto insurance.
• 1 year of home health experience preferred.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Initiate and revise the care plan as patient status changes.
• Regularly re-evaluate patients’ needs and adjust plans.
• Use health data to determine nursing diagnoses and plan goals.
• Educate patients and families about the plan of care and treatments.
• Identify discharge planning needs and coordinate before discharge.
