Job summary

Bridge Home Health & Hospice is seeking a registered nurse to plan, organize and direct home care services with emphasis on community health education. The role focuses on assessing patients in home settings, initiating and revising plans of care, and coordinating with families to meet individual and family needs.

The ideal candidate holds a California RN license, CPR certification and a nursing degree. At least two years of licensed experience in California and a preference for home health care

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience (California).
• Current licensure in state and CPR certification.
• Bachelor’s degree, with one (1) year of home health care experience preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates plan of care and revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis with patient/family input.
• Administers medications and treatments as prescribed by the physician and provides direct patient care.
• Counsels the patient and family in nursing needs and related areas.
• Provides health care instructions per assessment and plan of care.
• Identifies discharge planning needs and coordinates care prior to discharge.
• Acts as Case Manager to coordinate patient care for assigned caseload.
