Job summary

Mission Healthcare in North OC territory is seeking a dedicated Registered Nurse for full-time day shift in home health. You will perform initial patient assessments, develop care plans, and evaluate ongoing needs, in collaboration with families and physicians.

Candidates must hold a current RN license in California, have at least one year of acute care experience, and hold a BLS CPR certification. A Bachelor’s degree in Nursing is required; home health experience preferred.

Qualifications

Current and valid state RN license.
Minimum of 1 year of recent acute care in an institutional setting.
BLS with CPR certification.
Valid driver's license and auto insurance.
Bachelor’s degree in Nursing; home health exp preferred.

Responsibilities

Completes an initial assessment of the patient and family to determine home care needs.
Initiates the plan of care and revises as patient status changes.
Regularly re-evaluates patients’ needs.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan with goals based on nursing diagnosis.
Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
Counsels patient and family in meeting plan of care and related needs.
Provides health care instructions per assessment and plan.
Identifies discharge planning needs and implements prior to discharge.
